Personal record keeping
SymptomLedger is provided by Wild Logic Web LLC for adults 18 and older. By using the app you accept these terms and the Privacy Policy. You own your records and are responsible for their accuracy, backups, and sharing decisions.
Medical and legal boundaries
SymptomLedger is not a medical device and does not diagnose, treat, cure, or prevent any medical condition. It organizes your own observations. Consult a qualified healthcare professional for medical advice. It does not provide legal advice and is not affiliated with the VA or any government agency. Summaries are personal drafts calculated from selected records, not clinical assessments, official evidence, nexus letters, DBQs, or predictions of benefits or outcomes. Missing entries do not mean symptoms were absent. Review records before sharing.
Emergencies
This app is not monitored. For an emergency, call your local emergency number. In the U.S., call 911. For the U.S. 988 Suicide & Crisis Lifeline, call or text 988; veterans may call 988 and press 1.
Use and availability
Use the app lawfully and only store information you have permission to keep. We do not provide medical care, legal representation, or a managed record-retention service. Device failure, lost backups, uninstalling, or changing screen-lock protection can make data unavailable. Keep independent backups you control.
To the extent permitted by law, the app is provided as available without a guarantee of uninterrupted operation or fitness for a particular purpose. Nothing in these terms excludes rights or responsibilities that applicable law does not allow to be excluded. Updated terms will be provided with an updated date and renewed acceptance where needed.
